HHS Independent Agency Funding Availability – The St. Johns County Health and Human Services (HHS) Department will be accepting electronic applications for the Independent Agency Funding Program from April 22, 2019 through 3 p.m. on Friday, June 7, 2019. The Board of County Commissioners funds the St. Johns County Health and Human Services Department, which offers social support and assistance to needy members of the community.
Family Integrity Program (Community Based Care Division) provides foster and adoption services through a contract with the Florida Department of Children and Families.
Social Services assists clients by connecting them with a wide variety of resources to promote, protect, maintain and improve the health status of citizens.
Housing and Community Development is responsible for the promotion, development, preservation, and administration of affordable housing programs, initiatives and policies in St. Johns County.
Veterans Services Office assists veterans and their dependents in applying for benefits to which they are entitled under Federal or State Law or Regulations by reason of Active Military Service in the Armed Forces of the United States.
Health and Human Services Advisory Council
The volunteer Health and Human Services Advisory Council advises the Board of County Commissioners on health, mental health, substance abuse and social matters. They also review funding applications for local independent agencies that provide services to St. Johns County residents and make recommendations to the County Commission.
St. Johns County Health Department
Additionally, the County Commission supports the St. Johns County Health Department, an agency of the Florida Department of Health. The Health Department provides a wide array of services including certain Medical Services, Health Education, Public Health Mobile Center, Child Dental Services, Nutrition Counseling, Environmental Health, and Children's Services.
Mental Health Services
St. Johns County Mental Health closed effective July 29th, 2011. As a result, the State of Florida has identified a new mental health provider for St. Johns County. A few details are outlined below:
Provider: Community Mental Health services in St. Johns County are now being provided by Stewart-Marchman-Act Behavioral Healthcare (SMABHC), effective February 1, 2012. For information, appointments, and mental health records contact (904) 209-6200. Putnam-St. Johns Behavioral Healthcare mental health records can also be obtained by contacting SMABHC.
Services: SMBHC provides mental and behavioral health services as well as substance abuse, residential and detox treatment services.
Location: St. Johns County Health and Human Services Center located at 200 San Sebastian View, St. Augustine, FL 32084.
Americans with Disabilities Act – In compliance with the Americans with Disabilities Act, St. Johns County shall make available free sign language interperter services and other appropriate aids and services to customers or companions who are deaf or hard of hearing and request them. To request services, please call our office at (904) 209-6080. For questions in reference to Title II Procedures regarding the Americans with Disabilities Act, please contact the St Johns County Compliance Officer, Katie Diaz at (904) 209-0650. Records Requests: Former patients of St. Johns County Mental Health may request their mental health records by contacting the Health and Human Services Department at (904) 209-6030, and completing a request form. Please note that it can take up to 30 days to fulfill a health record request and there may be an associated fee.
